It is best to use our forms on a computer with Mac OS, Windows 7, 8 or 10 on it. Your computer should be running the latest versions of the following browsers:

• Chrome
• IE 11 (10 or below are not supported)
• Safari (Please note, to use Safari for LHI interviews you may need to change a setting in your preferences by taking the following steps: Open Safari> Choose “Preferences”, then select “Advanced”; next, deselect Stop plug-ins to save power)

Microsoft Edge and Mozilla Firefox are not compatible with most of our interviews. Please consider using one of the above internet browsers.

Your browser must accept cookies and window pop-up blocking should be disabled.

In order to edit and print assembled word processor documents, you need to have Microsoft Word or some other compatible program that can handle .rtf or .docx files.

In order to view and print assembled PDF forms, you need to have a current version of Adobe Acrobat Reader, which is available as a free download from Adobe.

Our forms are not currently supported for mobile or tablet use.
